How to register with the Practice
Please register with the Practice online, you will need an email address to do this. If you are unable to do this, please call into the surgery and a receptionist will help you.

If you wish to register you will need to complete a GMS1 and New Patient Questionnaire. We also need to see evidence of identity and proof of address and the person registering must submit their documentation to the surgery to be checked.
Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.
Pre-register online
If you are unable to come to the surgery with proof of address or ID, please upload your scans of photo identity and proof of address to ensure that your registration can be processed. After doing this, you will then receive a welcome email from the registration team within 1 week and details to your Patient Access account for online services.
Please note that we will hold your completed form for one month – After that period, if you have not submitted your photo ID and proof of address, your registration form will be deleted from our system.
Please note, if you are unable to complete this form online, please ask the Surgery for a paper copy of this registration form.
Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Please complete one form for each family member.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.
Organ Donation
